Trade Name ID Number, Name, Document Number, and Trade Name

Jurisdiction
To correct the jurisdiction under which the business was formed, choose a new jurisdiction from the drop-down menu. If the desired jurisdiction is not listed, select “Other”. If “Other” is selected, the jurisdiction must be entered in the space provided.
Historical purposes
Click the “Yes” button if the document is being delivered for filing for historical purposes and should not correct information currently of record.
Additional information
