completing a Statement of Trade Name Renewal of a Dissolved or Delinquent Reporting
Entity, Non-Reporting Limited Partnership or a Converted Entity pursuant to § 7-71-105 and §7-71-107 of the Colorado Revised Statutes (C.R.S.). The required
form/cover sheet must be used when submitting the document for filing. Information included in the document must be
typewritten or machine printed and must be in English. Mistakes may have legal consequences;
carefully review the information you provide.

incorrect and cannot be changed using this form, the appropriate form must be
submitted for filing to change or correct the information.
ID Number: The ID Number field will be pre-filled with the 11
digit number assigned to the record by the Colorado Secretary of State’s Business
Division. This information cannot be changed.
True Name: The True Name field will be pre-filled with the name
of the dissolved or delinquent reporting entity, non-reporting limited
partnership or converted entity exactly as such name is recorded by the Colorado
Secretary of State’s Business Division. This
information cannot be changed using this form.
Document Number: The Document Number field will be pre-filled with the
11 digit number assigned to the trade name by the Colorado Secretary of State’s
Business Division. This information
cannot be changed.
Trade Name: The
Trade Name field will be pre-filled with the trade name exactly as such name is
on record in the Colorado Secretary of State’s Business Division. This
information cannot be changed using this form.
Principal Address: The principal address may have been pre-filled. You
may edit this information.
Street Address:
A street address is required. Provide
the street name and number, including the suite, unit or apartment number, if applicable.
The city, state, and ZIP/postal code must also be provided. Any address outside
of the United States
must include the country and, if applicable, the province. The street address must be a physical address
and cannot be a post office box.

Mailing Address: A mailing address is optional. If the mailing address is different than
the street address, or mail cannot be delivered to the street address, provide
the address, including the city, state and ZIP/postal code where mail is to be

Address No Longer Different Than Street Address: If the mailing address is the same as the street
address, or the mailing address is no longer valid, mark the box.
Kind of Business: Briefly
describe the kind of business that is, or will be, transacted or the activities
that are, or will be, conducted using the trade name.

Additional Individuals Causing Delivery: Mark the box to include an attachment with the names
and addresses of additional individuals causing the document to be filed. The
attachment must provide the name of each additional individual, including their
