page for thirty days after the effective date of this filing.
ID Number:
The ID Number field will be pre-filled with the 11 digit number assigned to the
record by the Business Division of the Colorado Secretary of State’s Office.
This information cannot be changed.
Entity Name or True Name: The Entity Name or True Name field will be pre-filled
with the entity name or true name exactly as such name is on record in the Business
Division of the Colorado Secretary of State’s Office. This information cannot
be changed using this form/cover sheet .
Jurisdiction of Entity: The Jurisdiction field will be pre-filled with the
entity’s jurisdiction of formation exactly as such jurisdiction is on record in
the Business Division of the Colorado Secretary of State’s Office. This
information cannot be changed using this form/cover sheet .
Date of Resignation: Indicate the date on which the registered agent of
record resigned.
Registered Agent, Registered Agent Street Address and
Registered Agent Mailing Address: These fields will be pre-filled
with the current information exactly as it exists on record in the Business
Division of the Colorado Secretary of State’s Office. This information cannot
be changed using this form/cover sheet .
Delivery of Notice: The registered agent must
deliver notice of the change to the entity. Mark the box to affirm the
statement. If the box is not marked, the document will not be accepted. The
statement regarding delivery of notice of change is required by § 7-90-702
(2), C.R.S.

Delayed Effective Date (optional):
Leave this field
blank if you want the document to take effect immediately. The effective
date of this document may be delayed up to 90 days after filing with the Business
Division of the Colorado Secretary of State’s Office. If you do not want the
document to become effective immediately, enter in the box the date and, if
appropriate, time that you want the document to become effective. If only a
date is entered into the box, then the document will become effective at 11:59 p.m. on that date.
Mountain Time controls the filing date and

C.R.S. Pursuant to §
7-90-702, C.R.S., the resignation of
a registered agent is effective on the 31st day after the date that the
Statement of Change Regarding Resignation or Other Termination of Registered
Agent is filed in the records of the Business Division of the Colorado Secretary
of State’s Office. If the Statement of Change Regarding Resignation or Other
Termination of Registered Agent states a delayed effective date, the
resignation will become effective on the delayed effective date if the delayed
effective date is not earlier than the thirty-first day and not later than the
ninetieth day after the date the document is filed in the records of the
Business Division of the Colorado Secretary of State’s Office.
